Ozzy Osbourne's real cause of death is now revealed.

It's been two weeks since the music icon died at 76. The Black Sabbath lead singer died on July 22 at his home in Buckinghamshire, UK. His wife, Sharon, & their kids were with him.

According to his death certificate, Osbourne suffered a cardiac arrest outside of a hospital. Listed contributing conditions included acute myocardial infarction, coronary artery disease, and Parkinson's disease with autonomic dysfunction.

His occupation on the document read simply: "Songwriter, Performer and Rock Legend."

Final Days Marked by Music and Family

Osbourne's final public appearance out was just days from his death. He reunited with the first Black Sabbath members for a good show at Villa Park in Birmingham.

Following the concert, Osbourne was spotted in a video a relative shared online. He was dressed in a black T-shirt and headphones as he scrolled through his iPad. "Good morning," Osbourne said cheerfully to the video.

The rock icon had Parkinson's since 2003. The illness, mix with more health woes, made him pop in and out of the limelight for years. He and his wife would often talk openly about his health battles.

Kelly Osbourne's Tribute

Following the news of her father's passing, Kelly Osbourne wrote a heartfelt note to fans on Instagram. "I've sat down to write this a hundred times," she confessed. "But from the bottom of my heart, thank you. The love, support, and beautiful messages... have truly helped carry me through the hardest moment of my life."

She continued, "Grief is a strange thing-- it sneaks up on you in waves. I will not be ok for a while. But knowing my family are not alone in our pain makes a difference."

Kelly, who became engaged at her father's final concert, said she's holding on to "the love, the light, and the legacy left behind."

A Public Goodbye

The family held a private funeral at their estate, but fans in Birmingham had their own chance to say goodbye. Flowers covered Broad Street as mourners gathered near the Black Sabbath bench and bridge. Many sobbed as the music legend's coffin passed.

Born in Birmingham in 1948, Osbourne rose to fame as the voice of Black Sabbath before launching a solo career that spanned decades. His death certificate offers a fitting close to that legacy: "Rock Legend."
